Would I have to use DreamMaker?

Yeah im pretty sure you would just rip it from DreamMaker and then paste it onto the new ones I don't know how well that would work though

It depends if the maps changed, (I think)
so If you use the latest version, there's chances that Onsen town hasn't changed much,
so you can try to apply the patch t have a translated Onsen town.

That sounds good. However, I'm still confused about how DreamMaker works.

Please tell me if I'm wrong.

It rips the text from the maps in a plain text file without the variables and all the you-must-not-touch stuff. Then, how would I apply the translation to this text file? Manually? Or is there a way to apply some kind of "difference"?

I'd like to know this as well, since I suppose some of the stuff translated on the sections on this forum hasn't been applied to a release yet and I'd like to keep up with all the newer translations.

I've thinking about how to translate the new VH version (100829).

If I had those files:
1) Original "dialogues.txt" extracted by DreaMaker from the japanese VH 100727.
2) Translated "dialogues.txt" from english VH 100727*.

I could create an executable that generates a translations file. It would contain the information like: line "ˆê“x—¿—‚ð‰^‚яI‚¦" in map XX was translated to line "Hello".

And so, I could create another exe that applies those changes into the "dialogues.txt" from VH 100829. For example, looking for the line "ˆê“x—¿—‚ð‰^‚яI‚¦" in map XX and replacing it for "Hello"

Do you guys think it could work?

I'm guessing here, but I think it'd work as long as there aren't any lines with "double sense", if you know what I mean. I've heard a lot of japanese words mean different things.

Would replacing whole lines be a better approach here? Or is that what you meant?

I meant replacing whole lines and only in the same map.
